Requirements to Apply for Jackson Prime Benefits

To qualify for Jackson Prime Benefits, applicants must meet the following criteria:

• Residency: Must have resided in Miami-Dade County for at least 90 days
• Insurance Status: Must be uninsured.
• A valid email address is required for all communications regarding our application.
• Timely Document Submission: All requested information must be submitted within 30 days of the application date. Eligibility will be determined within 30 days of receiving your complete application. All official communications regarding your case will be sent to you via email or mailed to the address you provided on your application.
• Financial Assessment: Full cooperation with the financial assessment review process is mandatory. Failure to do so may result in the decline of your application.
• Other Program Ineligibility: Must not be eligible for other programs, such as Medicaid, or must have been previously rejected for coverage from such programs.
• Department of Children and Families Compliance: If required, the applicant must comply with the Department of Children and Families.
