The Jackson Prime program is for patients without medical insurance living in Miami-Dade County. If you have been enrolled in Jackson Prime in the last 6 months or are currently enrolled with an expiration date within the next 30 days, you can renew your enrollment through this online portal.
New Application
To renew, you must provide information about yourself, your Jackson Prime Medical Record Number, and any changes you have had since your last Jackson Prime enrollment.
To complete your application, you must upload proof of residency and any changes of income or citizenship/immigration status since your last Jackson Prime enrollment.

To continue, you must now provide your Jackson Prime MRN and the reciept number you were given when you last submitted your renewal application.
