CentroMed patients may begin their application for the sliding fee discount program online. To complete the process, you must visit your nearest CentroMed eligibility office to sign and finalize your application within 30 days of submission . You may upload required documents online, but final approval will only be given in person once all requirements are met.

Please note: submitting an online application does not grant automatic enrollment or qualification .
