This application is for patients without health insurance. If you have health insurance, you may qualify to apply for the Hardship Settlement Program. For more information and the requirements to apply, please visit the Atrium Health website at Atrium Health Financial Assistance or call Customer Service at 704-512-7171.

If you have a recent Atrium Health account number, please have it ready - you will need it for your application. If you are a new patient and do not have an account number, enter the word NEW when asked to enter your account number.
